APPLICATION PACKAGE CHECKLIST
Application requirements/components will vary depending on the type of grant and/or the country for which you are applying. It is important that you follow the general guidelines for application as well as the guidelines for the specific type of award, program, and/or host country.
Each of the items is explained in more detail on this website. It is the applicant’s responsibility to understand what is required for their application/program and to submit the appropriate documents by the stated campus or IIE application deadline.
A COMPLETE APPLICATION WILL CONSIST OF THE FOLLOWING ITEMS:

■ FORM 9A: References for Research/Study Grants (3): Three letters of reference are required. Form 9A is an optional coversheet that is generated when the referee prints the reference from the Online Application System.

■ FORM 9B: ETA Reference Forms (3) Other * Form 11: Creative and Performing Arts Supplementary Form and Materials ■ Original Transcripts from all degree granting institutions attended. See complete instructions for submitting transcripts.
ALL APPLICATION PIECES MUST BE SUBMITTED IN A SINGLE PACKAGE. To ensure that your complete application is reviewed, all materials/documents must submitted in one application packet.
If you are applying through your home campus, it is your responsibility to make sure the FPA has all your materials/documents by the campus deadline.
